Intuitive Intake and Complete Medical History

Once your initial consultation is scheduled and confirmed, you will receive intake forms and access to the Practice Better platform that you will use to submit intake forms, message with questions or concerns, and more.  

All visits are virtual (video visits) using this platform, and you can download the Practice Better app for your phone if you choose.  My clients report Practice Better is intuitive and easy to use.  

When your visit is confirmed, you will receive an email with forms to complete. I'm excited to work with you!
